Use this process to capture authorized representative information for EAP as well as add, edit or delete authorized representative records.
The Authorized Representative page displays.
In the Dates section:
Enter the Effective Begin Date.
Enter the End Date.
In the Authorized Representative Details section:
Select the authorized representative’s Prefix.
Enter the authorized representative’s First name.
Enter the authorized representative’s Middle name.
Enter the authorized representative’s Last name.
Select the authorized representative’s Suffix.
Enter the authorized representative’s Phone #.
Enter the authorized representative’s Organization Name.
Enter the Street #.
Enter the Fraction.
Select the Pre-Direction.
Enter the Str. Name / Rural Addr.
Select the Street Type.
Select the Post-Direction.
Select the Dwelling Type.
Enter the Address 2 / PO Box.
Enter the City (required).
Select the State (required).
Enter the Zip (required).
In the Authorized Representative Permissions section:
Select Yes or No for the Allow authorized representative to view data dropdown menu.
Select Yes or No for the Is the Authorized Representative 18 years or older? dropdown menu (required).
Enter the Duties to authorize for this person (required).
Click Add.
Click Save + Continue.
The name, address, contact and permission details are captured for the Authorized Representative.
