Authorized Representative - Details
Use this process to enter authorized representative details.
Process Steps:
The Authorized Representative – Details page displays.
- In the Dates section:
- Enter the Effective Begin Date (required).
- Enter the Reported On (required).
- Enter the date the verification was received in the Verification Received On field (required).
- Enter the date customer became aware of the details in the Date Change Occurred field (required).
- In the Authorized Representative / TANF Protective Payee Details section:
- Enter the First Name.
- Enter the Middle Name.
- Enter the Last Name.
- Enter the Phone #.
- Select the Relationship to Applicant from the dropdown menu.
- Select Yes or No from the TANF Protected Payee dropdown menu.

In the Authorized Representative / TANF Payee Address section:
- Enter the Organization Name.
- Enter the Street # (required).
- Enter the Str. Name / Rural Addr.
- Select the Street Type from the dropdown menu (required).
- Select the Dwelling Type from the dropdown menu.
- Enter the Address 2 / PO Box (required if Street # is not entered).
- Enter the City (required).
- Select the State (required).
- Enter the Zip (required).
- Complete the non-required fields as appropriate.
- In the Authorized Representative Permissions section:

Select the Type of Assistance from the selection menu (press and hold the CTRL key to select multiple options in this field).

Select Yes or No from the Allow Authorized Representative to view data dropdown menu.

Select Yes or No from the Is Authorized Representative 18 years or older? dropdown menu.

Select the Duties to Authorize for this person from the selection menu (required if Type of Assistance is SNAP. Press and hold the CTRL key to select multiple options in this field)..
- Click Save + Continue.
Results of the Procedure:
The name, address, contact and relationship details are captured for the Authorized Representative.
