Use this procedure to record disability details in the household.
The Disability Details page displays.
In the Client Information section:
Select the name of the individual in the household who is disabled from the Client Name dropdown menu.
In the Disability Dates section:
In the Effective Begin Date field, enter estimated date when disability started. Enter the date that the disability was reported to VDSS in the Reported On field.
In the Date Change Occurred field, enter the date of the most recent client disability change.
Enter the date when the client presented documentation to prove disability in the Verification Received on field.
In the Disability Information section:
Select the type(s) of disability from the Type of Disability box (required).
Select the form of documentation provided to verify the disability from the MA Verification dropdown menu. (enabled only for MA program)
Select the form of documentation provided to verify the disability from the SNAP Verification dropdown menu. (enabled only for SNAP program)
Select the form of documentation provided to verify the disability from the TANF Verification dropdown menu. (enabled only for TANF program)
Enter the MA Disability Begin Date (required if MA program is selected).
Enter the MA Disability End Date.
Enter the SNAP Disability Begin Date (required if SNAP program is selected).
Enter the SNAP Disability Review Date.
Enter the TANF Disability Begin Date (required if TANF program is selected).
Enter the TANF Disability Review Date.
Select Yes or No from the Reduced Ability to Work dropdown menu.
Select Yes or No from the Reduced Ability to Care for Child dropdown menu.
Select Yes or No from the Need someone in the home? dropdown menu.
Select Yes or No from the Permanent Incapacity dropdown menu.
Select Yes or No from the Mentally / Physically unfit dropdown menu.
Select Yes or No from the Has individual applied for SSI/SSDI? field.
Enter the date the application occurred in the SSI/SSDI Application Date field.
Select the current application status from the SSI/SSDI Application Status dropdown menu.
Select Type of Disability from the dropdown menu.
Select a Payment Code from the dropdown menu, if applicable.
Select Yes or No from the SSA/SSDI Denied disability in the past 12 months? dropdown menu (can only be selected if SSA/SSDI Application Status is Denied).
Select Yes or No from the Applicant alleges a new condition that has not been considered by DDS/SSA dropdown menu (can only be selected if answer to previous question is Yes).
Select Yes or No from the Applicant alleges his condition has changed or deteriorated dropdown menu (can only be selected if answer to SSA/SSDI Denied disability in the past 12 months? is Yes).
Select Yes or No from the Has the SSI payment now stopped? dropdown menu.
Select Yes or No from the Filed an appeal for denial? dropdown menu.
Select Yes or No from SSA has refused to reopen or reconsider claim? dropdown menu (enabled if answer to previous question is Yes).
Enter the Outcome of Appeal in the field (enabled only if Filed an appeal for denial? is Yes).
Select Yes or No from the Did the individual or individual’s spouse have a new medical condition or has the condition changed or worsened since the most recent application for disability was denied? dropdown menu (enabled for MA only).
Explain the change in Medical condition in the field (enabled if answer to previous question is Yes).
Select Yes or No from the Expedited Referral for Transfer to Rehab.
The Disability Determination section fields display in read-only.
If sending a Disability Referral:
Upload the following documents:
Disability Disclosure (SSA form 827) (required)
Document Type – Medical Information
Document Sub Type – Disability Determination Disclosure
Disability Report (SSA form 33368) (required)
Document Type - Medical Information
Document Sub Type – Disability Determination Report
Click Save + Send Disability Referral to trigger the DDS referral and proceed to the next page in the driver flow, if necessary. Proceed to the QRG for more information on Sending a Disability Referral.
Disability Information details of the household are recorded.
