Disqualification Details

Use this process to capture details about a SNAP disqualification for a customer.

Process Steps:

The Disqualification Details page displays.

In the Client Information section:

- Enter the First Name (required).
- Enter the Middle Initial.
- Enter the Last Name (required).
- Select the Suffix.
- Enter the SSN (required).
- Select the Sex from the dropdown menu.
- Enter the DOB (required).

The above fields will be read-only if the Add button was clicked to navigate to this page from the Disqualification Search / Summary page.

In the Disqualification Information section:

- Enter the Decision Date (required).
- Enter the Disq. Start Date (required).
- Select the Penalty Length from the dropdown menu (required).
- Enter the Number of Months.
- Select the Disqual Number from the dropdown menu (required).
- Select Yes or No from the Trafficking Offense dropdown menu (required).
- Select the Determination Reason from the dropdown menu.
- Enter the FIPS Code.

The above fields except Determination Reason will be loaded only if the edit icon was clicked for a row with disqualification data on it from the Disqualification Search/Summary page.

In the Disqualification Contact Details section:

- Enter the Location (Locality) Name.
- Enter the Title.
- Enter the First Name.
- Enter the Last Name.
- Enter the Phone Number.
- Enter the Phone Extension.
- Enter the State Code.
- Enter the Fax Number.
- Enter the Organization Name.

Click Save + Add.

Results of the Procedure:

The details about the SNAP Disqualification are recorded.
