Use this process to enter details on an EAP application.
The EAP Application Details page displays.
In the Components section, you can:
Select the Component Type (required).
Select the Crisis / Cooling Request Type.
Enter the Application Date.
Click Add.
In the Application section:
Select the customer’s Prefix.
Enter the customer’s First name (required).
Enter the customer’s Middle name.
Enter the customer’s Last name (required).
Select the customer’s Suffix.
Enter the customer’s Reading Language.
Enter the customer’s Spoken Language.
Enter the customer’s Home #.
Enter the customer’s Work #.
Enter the customer’s Cell #.
In the Correspondence Information section, enter the Preferred Method of Correspondence, then the Preferred Correspondence Cell # and Preferred Service Provider or Preferred Correspondence Email, if applicable.
At application or renewal, the electronic (email/text) Validated Method of Correspondence is a read-only field which is populated only after the customer has validated their new/changed Preferred Method of Correspondence using the verification code in CommonHelp. |
In the Physical Address section:
Enter the Street #.
Enter the Fraction.
Select the Pre-Direction.
Enter the Str. Name / Rural Addr.
Select the Street Type.
Select the Post-Direction.
Select the Dwelling Type.
Enter the Address 2 / PO Box.
Select Locality (required).
Enter the City (required).
Select the State (required).
Enter the Zip (required).
Click Validate Address / Get Locality to enable and populate the Locality from the dropdown menu in the physical address..
In the Mailing Address section:
Enter the Street #.
Enter the Fraction.
Select the Pre-Direction.
Enter the Str. Name / Rural Addr.
Select the Street Type.
Select the Post-Direction.
Select the Dwelling Type.
Enter the Address 2 / PO Box.
Enter the City (required).
Select the State (required).
Enter the Zip (required).
Click Save + Continue.
VaCMS will compare service addresses with service addresses for all cases where EAP status is approved/pending/closed and for the same EA component within the same program year.
