Use this process to change the case name on an EAP case.
The EAP Case Name Change page displays.
In the EAP Case Name Dates section:
Enter the Begin Date when the new case name takes over.
Enter the End Date when the current case name will end.
In the EAP Case Name section:
Select who will be the head of household on the case from the Name dropdown menu.
Select reason for the case name change from the Case Name Change Reason dropdown menu.
Click Submit.
The Relationship to Applicant field on the EAP Client Additional Information page is reset and will need to be updated for all applicable customers on the case when the case name is changed.
The EAP case name is changed.
