Use this process to capture additional client information relevant to EAP.
The EAP Client Additional Information page displays.
In the EAP Additional Information section:
Select the customer’s Name (required).
Select the Relationship to the Case Name (required).
If applicable, select Yes or No from the Disabled dropdown menu.
Select the Disabled Verification.
Select Yes or No from the US Citizen dropdown menu (required).
Select Yes or No from the Alien Status dropdown menu, if applicable.
Select the Alien Status Verification from the dropdown menu.
Select the household status from the Household Status dropdown menu (required).
Select the Absent Reason from the dropdown menu.
Enter the Estimated Date of Return.
Enter the Medicare Part B Amount, if applicable.
Select the Medicare Part B Payment Method from the dropdown menu.
Enter the Medicare Part D Amount, if applicable.
Select the Medicare Part D Payment Method from the dropdown menu.
Enter the amount for the Community Based Care Patient Pay Amount.
Click Add.
In the EAP Disqualification section:
Select Yes or No from Disqualify household for selling fuel or merchandise purchased by the EAP dropdown menu.
Enter the Disqualification End Date.

If the question Disqualify household for selling fuel or merchandise purchased by the EAP? is marked as Yes, then the entire household will be denied for all pending service requests. |
Click Save + Continue or Save + Previous.
Additional client information has been added to the EAP case.
