Use this procedure to record immunization information for a customer.
The Immunization - Details page displays.
In the Client Information section:
Select the appropriate customer from the dropdown menu of all available customers in the case.
In the Immunization Dates section:
Enter the Effective Begin Date of the immunization record (required).
Enter the date the verification for the immunization record was received in the Verification Received On field (required).
In the Immunization Information section:
Select Yes, No or Exempt from the Is client immunization record current? dropdown menu (required).
Select the source of verification of the customer’s immunization record from the Verification Source dropdown menu (required).
Select Yes or No from the Has household member received all immunizations required for his / her age? dropdown menu.
Select Yes or No from the Child has received at least one dose of each of the required immunizations as appropriate for the child’s age dropdown menu.
Select the Exemption Reason from the dropdown menu, if appropriate.
Select the source of verification of the exemption from the Verification Source dropdown menu.
Enter the Next Immunization Due Date field.
Click Save + Continue.
The customer's immunization record details are recorded.
