Life Insurance Details
Use this procedure to enter a customer’s life insurance information.
Process Steps:
- The Life Insurance – Details page displays.
- In the Individual Information section:
- Select the customer’s name from the dropdown menu (required).
- In the Life Insurance Dates section:

- Enter the Effective Begin Date (required).
- Enter the End Date.
- Enter the Reported On date (required).
- Enter the Date Change Occurred (required).
- Enter the Verification Received On date (required).
- In the Life Insurance section:

- Enter the Insurance Company Name (required).
- Select a Verification (required).
- Select a Type (required).
- Enter a Policy # (required).
- Enter the Policy Acquired On date.
- Select Yes or No from the Is Life Insurance Jointly Owned? dropdown menu (required).
- In the Life Insurance Company Address section:

- Enter the Street #.
- Enter the Fraction.
- Select the Pre-Direction.
- Enter the StrName/Rural Addr.
- Select a Street Type.
- Select a Post-Direction.
- Select a Dwelling Type.
- Enter the Address 2/PO Box.
- Enter the City.
- Select a State.
- Enter the Zip Code.
- Enter the Phone #.
- In the Life Insurance Value section:

- Enter the Face Value (required).
- Enter the Net Cash Value (required).
- In the People Insured Through Policy Information section (the name selected in the Client Information Section cannot be selected here):

- Select the People insured through this policy.
- Select the Someone outside the home checkbox if one or more individuals on the policy resides outside the home.
- In the Resource Availability section:
- Select an Availability Status (required).
- Click Save + Add Life Insurance to save the resource record.
- Repeat steps 1 through 9 to add additional resources.
- Click Save + Continue.
- If the Life Insurance is jointly owned, the Life Insurance – Joint Ownership tab appears:

- If the joint owner is in the home, select the Joint Owner name from the dropdown menu under the In-household Joint Owner Information section.
- If the joint owner is outside the home, enter the joint owner’s Name and other information in the Outside-household Joint Owner Information section.
- Select a Joint Ownership verification from the dropdown menu.
- Enter the Joint Ownership Percentage Share of Resource.
- Enter information in the remaining fields if necessary.
- Click Add.
- Click Save + Continue.
Results of the Procedure:
The life insurance information has been recorded.
