Use this procedure to enter application data as part of the Rapid Data Entry process.
The EA Non-Financial Information page displays after completing the MA Non-Financial Information page.
The Energy Assistance page displays.
In the Cooling Assistance section:
Select the client's Living Situation from the dropdown menu.
Select the client's Cooling Need from the dropdown menu.
Select the client's Equipment from the dropdown menu.
Select the client's Equipment not in working condition from the dropdown menu.
Select the Equipment Owner from the dropdown menu.
Choose Yes or No from the Received CA in last 12 months? dropdown menu.
Choose Yes or No from the Help from other sources? dropdown menu.
If other help is received, enter Who is helping? in the field.
Choose Yes or No from the Heating Expense dropdown menu.
Select the Fuel Type from the dropdown menu.
In the Fuel Assistance section:
Select the client's Living Situation from the dropdown menu.
Select the client's Equipment Used from the dropdown menu.
Select the client's Fuel Type from the dropdown menu.
Choose Yes or No from the Responsible for paying heat? dropdown menu.
Choose Yes or No from the Fuel delivered to home? dropdown menu.
Enter the Fuel Tank size in gallons.
In the Crisis Assistance section:
Select the client's Living Situation from the dropdown menu.
Select the client's Emergency Need from the dropdown menu.
Select the client's Fuel Type from the dropdown menu.
Select the Equipment Owner from the dropdown menu.
Choose Yes or No from the Another Heat Source? dropdown menu.
If there is another heat source available, select Another Heat Source Name from the dropdown menu.
Choose Yes or No from the Have Temporary Shelter? dropdown menu.
Enter the Fuel Tank size in gallons.
Enter the amount of Fuel Left.
In the Energy Provider section:
Select the provider's Assistance Type from the dropdown menu.
Select the Name on the bill.
Enter the client's Street Address.
Enter the City (required).
Select the State (required).
Enter the Zip (required).
Select the Company Name for the energy provider.
Enter the client's Account Number.
Select the Person Responsible for the bill from the dropdown menu.
Choose Yes or No from the Automatic Payment dropdown menu.
If the company name is "OTHER", enter the following information for the company:
Street Address
City (required)
State (required).
Zip (required).
Phone Number
Click Save + Continue.
Rapid Data Entry process continues to the Liquid Resources page.
