Request MA Replacement Card

Use this procedure to search for an individual client associated with a particular case, request a Medicaid card replacement and perform an MMIS inquiry (TPL / BENDEX inquiry).

Process Steps:

  1. From the Left Navigation menu, click Replacement Card(s), then MA Replacement Card.
  2. In the Client Search Criteria section:

 

Each client search must contain at least a Client ID, SSN, or Last name. Enter information in some or all of the fields to decrease or expand the search range as appropriate. The more criteria entered the more specific the search results will be.

 

  1. Click Search.
  2. To view specific client details:

 

Each row in the Client Search Results section contains a document icon. Clicking any icon will take you to the Document Search Results screen.

BENDEX information might not be available until the monthly BENDEX process runs in MMIS approximately the third week of the month.

 

  1. To request a replacement Medicaid card:

 

When requesting a replacement Medicaid card, the VaCMS checks for enrollee ID and also checks if the client is active on that case. This check will not be performed using transactions to MMIS but using EDBC tables. Also client needs to be active ON that same case.

If the client is not active on the case, an error message will be shown to the worker – “Medicaid card request cannot be made, as the client is not active on this case”.

 

Results of the Procedure:

A client search is successfully completed.
