SNAP Replacement Card

Use this process to inquire on information about a SNAP case, and issue a replacement card.

Process Steps:

The SNAP Replacement Card page displays:

1. In the Case Search section:
   • Enter the Case # (required).
   • Click Search.

2. In the Replacement Card Information section:
   • Select the Issue To dropdown menu (required).
   • Select the Reason dropdown menu (required).
   • Select the Issuance Method dropdown menu (required).
   • Click Submit.

Results of the Procedure:

The process for ordering a replacement card is initiated and the status is displayed.
