School Enrollment Verification Form

Use this process to generate the School Enrollment Verification form.

Process Steps:

Navigate to the School Enrollment Verification Form page through the Search Case Information and Search Document Information pages. The School Enrollment Verification Form page displays.

In the School Enrollment Verification Information – Addressee Details section:

- Enter the School Name (required).
- Enter the Street # (required).
- Enter the Address Line 2 / PO Box (required if Street # is not entered).
- Enter the City (required).
- Select the State (required).
- Enter the Zip (required).
- Complete the non-required fields as appropriate.
- Enter the Attn / Care of Information.

In the School Enrollment Verification Information – Case Details section:

- Enter Pupil 1 Name (required).
- Enter Pupil 2 Name and Pupil 3 Name if necessary.

Click Preview to view a PDF preview of the form.

The selected form displays in a pop-up window as a PDF file and is auto-populated with the information previously entered into the VaCMS.

Click the print icon in the upper left corner of the PDF to print the form.

Click X in the upper right corner of the PDF to close the pop-up window.

Click Submit to send the form to be printed.

Results of the Procedure:

The School Enrollment Verification form is generated.
