Search for a Referral Agency

Use this procedure to search for referral agency information.

Process Steps:

1. From the Left Navigation menu, select Referral Agency – Inquiry.
2. The Referral Agency – Inquiry page displays.
3. In the Referral Agency – Search Criteria section:

• • Select a Locality (required).
  • Enter an Agency Type.
  • Enter an Agency Name.

4. Click Search.
   • The search results appear below.

5. To edit the existing record:

   • Click the pencil icon to view / edit the referral details on the Referral Agency – Details page.

6. To enter a new record
   • Click Save + Add Referral Details to access the Referral Agency – Details page.

Results of the Procedure:

You have successfully located the referral.
