Use this procedure to view and generate a verification checklist to send to the customer.
From the Left Navigation menu, click Data Collection then Miscellaneous, then Verification Checklist.
The Verification Checklist page displays.

The Verification Checklist page displays verification checklists for a case / Individual eligibility and allows user to extend Verification Checklist due date. The page displays the verification for the non-financial and financial eligibility criteria for which the valid verifications are not received. |
In the Case Information section:
Review the case information including:
Case Name
Case Number
Case Action
Case Status
In the Verification Checklist Information section:
Review the VCL information including:
Individual - name of person with a pending verification.
TOA - the Type of Assistance.
Pending Verification - the items that are missing required verification.
Valid Verification Documents - items that are acceptable forms of verification.
Verification Due Date - date by which verification document is required.
Additional Comments (Medical Assistance Only) - Additional comments that can be displayed on the VCL.
Enter a new date into the VCL Extension Date field.
Provide additional information on the extension in the Comments field.
Click the Trigger VCL button to create a notice to send to the client.
Click Submit.
A Verification Checklist for the customer is generated or the customer's verification deadline is extended.
