Use this procedure to view previously generated forms.
From the Left Navigation menu, select Forms, then View History.
The View Forms History page displays.
In the Search Criteria section:
Select the Worker radio button to search for forms by worker name. This radio button is selected by default.
Select the Locality from the dropdown menu when the Worker radio button is selected. This dropdown menu is only enabled for those with security access to view previously generated forms statewide.
Select the Supervisory Unit from the dropdown menu when the Worker radio button is selected. This dropdown menu is only enabled for those with security access to view previously generated forms for any unit in the locality.
Select the Worker Name from the dropdown menu when the Worker radio button is selected. This dropdown menu is disabled for workers and pre-filled with the name of the worker who is logged in to the VaCMS. This dropdown menu is only enabled for supervisors, who are able to select a worker in their unit to view their previously generated forms.
Select the Case radio button to search for forms by case number.
Enter the Case Number when the Case radio button is selected.
Enter the Vendor ID when the Vendor radio button is selected.
Enter the Correspondence ID.
Enter the Tracking Number when the Tracking radio button is selected.
Enter the Appeal ID when the Appeal radio button is selected.
Select the Form Name from the dropdown menu. This dropdown menu defaults to All.
Enter the first month, day, and year of the date range to view the previously generated forms in the Generated Between field. The date can be entered manually using the mm/dd/yyyy format or by clicking on the calendar icon and selecting a date.
Enter the last month, day, and year of the date range to view the previously generated forms in the And field. The date can be entered manually using the mm/dd/yyyy format or by clicking on the calendar icon and selecting a date.
Click Search.
In the Search Results section:
View the previously generated forms which are displayed by Suppressed, Mode, Form Name, Correspondence ID, Case Number (if the form is associated with a case), Case Name, Vendor ID (if the form is associated with a vendor), Generated Date, and Printed/Sent Date.
Click the link for the previously generated form to view.
The selected form displays in a pop-up window as a PDF file and is auto-populated with the information previously entered into the VaCMS. Click the print icon in the upper left corner of the PDF to print the form. Click X in the upper right corner of the PDF to close the pop-up window. |
The previously generated form is viewed.
