Use this procedure to build an employee profile.
From the Left Navigation menu, select Manage LDSS Resources, select Build Profile, then select Employee.
The Build Employee Profile page displays.
In the Employee Information section:
Enter the User ID of the employee or click the search icon and proceed to Search for User (pop-up window) to search for the employee information (required).
The Employee Information section populates with information that is provided when the internal user account is added by security.
Review the information auto-populated and update or enter information in the below mentioned fields as appropriate.
Enter the First name of the employee.
Enter the Last name of the employee.
Enter the phone number where the employee can be reached in the Phone # field and extension number in the x field.
Enter the fax number in the field Fax #.
Enter the Email address of the employee.
Enter the employee’s unit in the Associated Unit field or click the search icon and proceed to Search for Unit Profile (pop-up window) to search for the unit profile (required).
Select the Hospital that employs the worker if the profile is for a Hospital Based Worker.
Select the Job Title from the dropdown menu (required).
In the Additional Information section:
Select the Language from the list box.
Select the Default Type from the list box, if applicable. This field is necessary for default workers only.
Note: Some fields allow for multiple selections.
Select the checkbox next to the type of worker.
Select the DMAS Appeals Expertise from the dropdown menu.
In the Supervisor Review Criteria section:
Select the percentage of cases that the worker authorizes that need supervisor authorization from the Superv. Auth % field.
Click the checkbox next to the Target Program for supervisor review.
Click the checkbox(es) next to the Target Data Collection / EDBC Determination group(s) for supervisor review.
Select an option from the Living Situation (EAP Only) list box.
Select an option from the Heating Equipment Type (EAP Only) list box.
Select an option from the Crisis Assistance Requested list box.
Select an option from the Cooling Assistance requested list box.
Click Submit.
An employee profile is set up.
