Use this procedure to enter information about the client’s address.
The Register Address page displays.

There can only be one physical address, which is required, and one mailing address, which is optional. If a mailing address is not entered, correspondence is sent to the physical address. |
Select Physical or Mailing Address next to Address Type, if appropriate.
In the Address Information section:
Enter the Street # (required).
Enter the Address Line 2 / PO Box (required if Street # is not entered).
Enter the City (required).
Select the State (required).
Enter the Zip (required).
Select the Verification (required).
Complete the non-required fields as appropriate.

A locality lookup is required the first time an address is entered. The locality lookup consists of the VaCMS recommending the proper locality [Federal Information Processing System (FIPS)] based on the household’s address. After a lookup is completed once, the worker can bypass the locality lookup and manually enter the locality code by selecting No in the Is the locality lookup required? dropdown menu. |
Click Validate Address / Get Locality.
The Matching Address Results pop-up window displays.

Confirm that the locality code of the matching address is served by the Local Department of Social Services (LDSS) registering the application or waiting list screening. |
If potential matching addresses exist:
Review the potential matches.
Click the radio button of the matching address.
Click Populate Address / Locality.
The Matching Address Results pop-up window closes.
Click Add on the Register Address page.
Click Save + Continue to proceed to Register Client.
If there are no potential matching addresses:
The Matching Address Results pop-up window displays no results.
Click Cancel and the Matching Address Results pop-up window closes.

The Is the locality lookup required? field defaults to No when the VaCMS does not find a match for the address entered. |
Select the Locality.
Click Add.
If the address is located in a FIPS that is not served by the Local Department, proceed to Deny an Application / Waiting List Screening.
If the customer address is the LDSS address:
Enter the city and zip code of the local office.
Click Populate LDSS Address.
For MA and MA with Resource Assessment applications
Select Yes or No from the Is this the physical address for a nursing, medical, correctional, or assigned living facility? dropdown menu (required).
Select the state the customer lived in from the What state did you live in prior to entering the nursing, medical, correctional, or assisted living facility? dropdown menu, if applicable.
Select the locality the customer lived in from the In what locality did you last reside (in the community) prior to entering a nursing, medical, correctional, or assisted living facility? dropdown menu, if applicable.
Select Yes or No from the Is the Primary Applicant Homeless? dropdown menu.
In the Mailing Address Other Information section:
Enter the Begin Date that the client started using this mailing address. This date is required and defaults to the current date.
Enter the End Date that the client will no longer use this address as a mailing address. This field is required when listing a mailing address and must be either the current date or a future date.
Click Save + Continue.
Proceed to Register Client.
The address information is recorded and a unique number beginning with “T” is assigned to the application or waiting list screening.
