Use this procedure to enter the name, demographic, and other information for each client in the household.

At this stage in the Registration process, the application or waiting list screening has now been assigned an Application / Screening Number. The number begins with a "T" and is displayed at the left of the page header. |

The name of the Applicant entered earlier is auto-populated in the Individual Information section of the page. Information must be entered for every member of the household for File Clearance purposes. You will be prompted to add other individuals to the case later in the Registration process. |
The Register Client page displays.
In the Client Name section:
Enter the First name (required).
Enter the Last name (required).
Complete the non-required fields as appropriate.
In the Demographic Information section:
Select the Gender (required).
Enter the DOB (required).

Press and hold the CTRL key to select more than one item from a list. This is useful to select multiple items for Race or Language, if necessary. |
Select the SSN.
Select the Primary Race (required).
Select the Ethnicity (required).
Select the Primary Spoken Language (required).
Select Yes or No from the Is this the individual for whom the resource assessment is being requested dropdown menu (required when program is Resource Assessment Only or Resource Assessment and Medical Assistance).
Complete the non-required fields as appropriate.
In the Other Information section:
Complete the non-required fields as appropriate.

If the client does not have alias names, clicking Next sends the first name, last name, gender, and date of birth, and social security number (SSN), if available, to SPIDeR to see if the client is known to the Virginia Department of Social Services (VDSS). |
Click Save + Continue.
If the use of an alias name has been indicated, proceed to Register Client Alias Name.
If the use of an alias name has not been indicated, proceed to View File Clearance Results.
The client information is recorded.
